The honest answer to "does it work?" is: yes, within clearly defined limits. Cryolipolysis is not a weight loss treatment. It is a body contouring tool for stubborn fat that doesn't respond to diet and exercise. Understanding this distinction is the single most important thing you can know before booking.

The science, briefly

Fat cells (adipocytes) are more sensitive to cold than other cell types. At approximately -5°C, fat cells begin to crystallise while surrounding skin, muscle, and nerve tissue remains unaffected. This selective vulnerability is what makes cryolipolysis possible.

The procedure applies controlled cooling to a specific area through an applicator that creates gentle suction. After 30–60 minutes, the cooled fat cells undergo a process called apoptosis (programmed cell death). Over the following 6–12 weeks, your lymphatic system naturally clears these dead cells from the body.

The result: a measurable reduction in fat layer thickness, typically 20–25% per treated session.

What it will and will not do

Will do:

  • Permanently reduce the number of fat cells in the treated area
  • Produce visible, measurable improvement in body contour
  • Treat stubborn areas resistant to diet and exercise

Will not do:

  • Cause significant weight loss (fat cells are a small percentage of body mass)
  • Tighten loose or sagging skin
  • Replace the effects of diet and exercise on overall body composition
  • Treat all body shapes equally (results vary based on fat layer thickness and skin laxity)

What the treatment feels like

I think it's worth being honest about this because you'll find very different accounts online.

The first 5–10 minutes are uncomfortable. The applicator creates strong suction, and the cooling is intense, most people describe it as an aching, burning cold. This is the most difficult part.

After about 10 minutes, the area goes numb. Once that happens, most patients are comfortable enough to read, work on their phone, or nap. The session runs for 30–60 minutes per applicator placement.

Immediately after, the treated area looks and feels like a firm block, which Dr. Bandara or a therapist will massage for approximately 2 minutes. This massage is not optional; studies show it significantly improves results by accelerating the breakdown of the crystalised fat. It is uncomfortable, but brief.

Recovery and results timeline

There is no downtime in the surgical sense. You can return to your normal activities immediately. Common post-treatment experiences include:

  • Temporary numbness, tingling, or aching in the treated area (normal, resolves within days to weeks)
  • Mild swelling or redness (resolves within hours)
  • Occasional paradoxical fat hypertrophy (PAH), a very rare complication where fat increases rather than decreases. This is extremely uncommon and can be treated.

Results timeline:

  • 4–6 weeks: first changes visible
  • 12 weeks: full result (fat cells are now fully cleared)
  • 6 months: optimal result with good lifestyle maintenance

How many sessions do I need?

For most areas with moderate fat accumulation, 1–2 sessions provide satisfactory results. Areas with more significant fat may benefit from a third session, assessed at the 12-week review.

Why I'm honest about the limitations

Every clinic in Sri Lanka offers some version of "body slimming." Many overstate results. I won't.

Cryolipolysis works very well for the right patients, those who are at or near their target weight, have specific areas of stubborn fat, and understand what they're asking the treatment to do. It does not work well as a motivational tool for people who are hoping it will replace lifestyle changes.
